Djimon Hounsou (A Quiet Place: Day One) may have sworn an oath to do no harm, but in Twisted, he’s more than willing to break it—violently—when Lauren LaVera (Terrifier franchise) crosses his path.

The gory horror thriller arrives on Digital February 6 via Republic Pictures, and the newly unleashed trailer promises blood-soaked punishment and surgical terror in equal measure.

Directed by Darren Lynn Bousman (Saw II–IV, Spiral), Twisted is scripted by Jonathan Bernstein and James Greer (Unsane) and plays like a modern morality tale dipped in viscera.

LaVera stars as a millennial grifter who bankrolls her lifestyle by leasing luxury New York City apartments she doesn’t actually own, scamming unsuspecting renters who never see it coming. But when her scheme collapses, she finds herself abducted by a surgeon (Hounsou) with a deeply personal—and horrifying—agenda.

The cast also includes Mia Healey (The Wilds), Gina Philips (Jeepers Creepers), Jacob Lukas Anderson (aka rapper Prof), Neal McDonough (Minority Report), and Alicia Witt (Longlegs).

Longtime Saw collaborator Mark Burg produces under the Twisted Pictures banner alongside Lee Nelson and David Tish of Envision Media Arts. Bernstein and Greer also serve as executive producers with Lukas Behnken, Trevor Gerszt, Rocky Savio, Curtis Lane, David Gendron, and Viviana Zarragoitia.

Twisted is rated R for “strong/bloody violent content, gore, sexual assault, suicide, language, and some sexual material”—a content warning that doubles as a promise for fans of brutal, body-driven horror.
